Reading sensitive information in medical records can be an unexpectedly distressing experience which can raise a host of difficult emotions. Entries in medical records were written for the purpose of recording key information for the people involved in caring for you/your relative and can therefore feel impersonal and stark in some instances. The tone can appear impersonal and some of the information can feel overwhelming.

We ask that you therefore consider this before you read entries and think about what support you might need if there is something contained in the record which you find upsetting.
